MEMBERSHIPS

   All Memberships are for a 6 month minimum term. No exceptions.  Memberships cannot be combined with other specials or discounts.  Monthly Memberships must be used each month and do not roll over.  If not utilized within the monthly billing cycle (see below) the month’s services/discounts/ benefits will be forfeited by the member.  Monthly Membership dues cannot be supplemented by any other promotional vendor programs such as Aspire or Alle Rewards.  

The monthly Membership starts the day you sign up for your Membership, and renews on that date each month. Your Member discounts and benefits renew on that date as well. For example, if you sign up on March 17th, 2022, you have until April 16th, 2022 to use your monthly Member payment benefit. 

All Memberships are non-transferable and non-refundable. You may receive a gift card for the value of 1-month of your active Membership to gift to someone else (this will be a one-time courtesy). If clients choose this option, they may not come in prior to the next billing cycle.  If a client decides to cancel Membership prior to the signed agreement, they will be charged 30% of the Membership monthly fee by  the number of months remaining in the term of the Membership.  

To cancel a Membership, a 30 day written notice is required and cancellation requests  must be sent to info@designmemedspa.com.  Cancellations can only be processed in person, otherwise, Membership will remain active.  Any Membership fee that is returned for insufficient funds, incorrect card number or card information is considered in arrears and due to Design Me Medical Spa, LLC. 

If a Membership is considered in arrears, Membership services and discounts will not be available for use until balance is paid up to and including the current month due. Membership fee cannot be offset by any in store credit and must be paid by a Visa/Mastercard/American Express/Discover. Monthly Membership fees are drafted on a 30 day rolling basis and are typically withdrawn on the same calendar day each month. An active credit card must be kept on file. 

 Particular appointment availability or provider availability is not and can not be guaranteed. Design Me Members agree to abide by Design Me Medical Spa Policies. Membership payments made are not refundable.
